The original authors and I get different results when we, apparently, build the user manual in exactly the same way. When I do it there are no running headers to the pages either of main.pdf or userManual.pdf.  

You need to generate main.pdf to get the via latex and bibtex to share the citations with userManual.pdf.

I thought the running heads were turned off when using

\pagestyle{plain} % remove running headers

at line 97 of userManual.tex. I tried fiddling with this and loading the booktabs package at line 23 but it appears to make no difference.

Life is too short -- I'm now 75 and it's seeming shorter by the day.

If anyone feels like contacting the Algorithms editor when they find out how to get the headers back, please drop him a line and s/he will update the files accordingly.

The pdf files currently provided are those supplied by the author and contain the running heads.
